Please complete all relevant sections of the form, and send it together with the required documents to Prescient via e-mail hedge@prescient.co.za. Application forms must be submitted before 14h00, five business days before the preceding month end. Proof of payment must be sent by the end of the month preceding the deal in date. The following supporting documents must be submitted with the application:
- FICA Documentation
- Proof of Bank Account Details
- Completed Compulsory Tax Annexure.
